Although I'm quite sure one of the two is much more frequently used than the other. As a person name, it's definitely polyglot (as in, "He is a real polyglot, he speaks six languages.". Not sure about the -ism noun.
(The reason for this is that polyglot is a Greek word in origin, whereas multilingualism is Latin - there are quite a few cases of a Latin and Greek word co-existing, although I can't think of any at the moment.
