Either he injected them after death and then posed their bodies (including the, um, insertion, and I'm trying REALLY hard not to think about the role rigor mortis played here... ) or they were having sex up till the moment of their death (highly unlikely in terminal cancer patients, especially lung cancer) and hopefully the plasticizer didn't sting too much going into them.
