I think it just depends on how bad the original infection necessitating the procedure is, as well as the location of tooth and the skill of the dentist.
The first one I had was agonizing and if they'd offered me Vicodin I'd have taken it eagerly. The second one hurt some, but not so much I that I needed anything stronger than over the counter painkillers to deal with the discomfort.

I was awake for a bottom one and the novocaine didn't take. After it was done, though it was fine.
