in truth, it was likely a little bit of both, as is usually the case.
I don't expect us to ever go back entirely to self-pay. But I think that shifting some attention away from "THE INSURANCE ISSUE" could be fruitful as well. Fix some problems that make medical costs high NO MATTER WHO IS PAYING, and it may alleviate the difficulty of "fixing" insurance.
