most likely collusion or at least benign neglect between the three major players - CIA, Mafia and the FBI (meaning Hoover).
Interestingly, both the CIA and the Mafia were former allies of the Kennedy family and collaborators in both the Bay of Pigs and the attempts to assassinate Castro. Whereas Hoover always hated them, starting with Joe Senior.
