The adjusted list judges a film by how many people actually went to see it in the cinema on release. Considering the much lower population of the world when AVATAR came out, the sheer popularity of GONE WITH THE WIND is impressive.
OTOH, the film was reguarly re-released and re-shown in cinemas many times for decades. I think you can even catch the odd cinema showing it now, and all money spent on tickets to it still goes towards the international gross. Same for STAR WARS (which is #2 I believe). If AVATAR is still being periodically re-released in 70 years time, it may well have toppled GWTW's record, but I doubt it.
AVATAR, has made a shedload more money at today's prices, which is what is important here. Given that the movie's budget, whilst huge, was about the same as TITANIC's adjusted for inflation, this is still an impressive achievement.
