Rand burned his face in the battle at the end of tEotW, Ch. 51. (Wish I had my book handy...) He channels a spray of fire at him after severing his connection with the Eye. Ba'alzamon (Ishamael) is then absent from the series for quite some time, making Rand think he has already defeated the Dark One. He comes back in tGH with scars on his face and wearing a mask.
