I think they should have coordinated what each cover would show


I'm pretty sure they (she) did come up with 12 concepts and artists right away (I doubt she carries on this project month after month, she's the main AD for Tor - she probably worked on most of the covers in parallel at some point), but Irene seems to rely heavily on Jason to validate the concepts, and Jason is fan very much into the triva/plot/characters, not really as much into themes etc. If you read his paragraph she quoted, you see where the flaw came from, that is he's the one who turned the Cleansing in her mind into a great Rand moment, oblivious to the team work aspect.
