There is also Min's viewing of Elayne in TGH, said there was a red hot poker and an axe in Elayne's future but far into the future, I doubt that means after the LB. Moghedien would be a good canidate for that. Also if you think about the loop holes about the her babies being born ok, maybe Moghedien has Elayne and is torturing her or something, maybe around the time Mat wants to see her, like after she sends her first letter.
