We don't know, but a lot of readers have theorized she put it together based on her basic Tower knowledge about the Forsaken (Sammael has a scar, he's blond and short - and as we know he didn't use Mask of Mirrors, so it's how he looked as Lord Brend) and what she gathered from Loial. It sounds extremely likely she managed to get a look a Sammael, or to get a good physical description from someone who saw him. She was already almost convinced a Forsaken was in Illian, and could inquire about dreams, or listen to stories about those etc.

Hmm, I can't find the quote, but wasn't there something somewhere about there being no surviving description of any of the forsaken? For example, Rand says he recognises them from Lews Therin's memories and Asmoadean's descriptions. If Moirane had a description of any of them, wouldn't she share it with Rand? And Cadsuane is surprised Rand is able to recognize Semirhage.


I don't recall her addressing this in the follow up POVs, but she had been considering for quite some time that Rand was going off the deep end. The fact that it turns out he truly did have ancestral memories and a Forsaken actually confirmed it was funny to me. I would say it shut her up, but it would take more than a minor point to make her think twice lol.
