I would place Graendal ahead of Demandred as she was a super badass fighting at Shayol Ghul as well as throwing Arad Doman and Shara into chaos ... The latter may have enabled Demandreds rise there... She compelled key people throughout the series and to be honest, she was just a more consistent character throughout the series. I may change my mind on her v. Demandred's ranking once we see the behind the scenes stuff on Demandred though.
Other than that I pretty much agree with your rankings for the Forsaken.
As for the Light side I'd rank them somewhat differently than you did ... But only 1 different person.
1. Rand
2. Mat
3. Nynaeve
4. Perrin
5. Egwene ... I place her higher because all of her actions were during the series... Also she unified the WT, without which the fight would have been lost, she purged the BA and is the only character other then Rand to kill off 2 Forsaken.
6. Moiraine
7. Lan ... Great deeds, but they were isolated to the Last Battle for the most part ... Stud warrior, but ultimately his actions were limited in scope.
8. Verin ... Uncovering the BA and passing the information along was HUGE
9. Elayne
10. Aviendha ... She is higher IMO because she also was pivotal in changing the course of her vision for the Aiel which ultimately help Rand win at the LB
11. Cadsuane (moved from 9 to 11)
12. Thom
13. Loial
14. Min ... Her visions and her studies pretty much gave Rand the answers to how to beat the DO. Other visions were key throughout the series helping save the day a number of times, including with Tuon during the last battle.
Other than that I pretty much agree with your rankings for the Forsaken.
