Selfish or Inconsiderate behavior = Cthullu doesn't care about mere mortals. My impression of the Old Ones is not that they are inimical so much as profoundly alien and indifferent to humanity.

Tyranny / Abuse of power = Sauron is the ultimate tyrant, who created the One Ring that inevitably gets misused, and counsels even great and wise rulers to fall into tyranny.

Out of Touch mentality = I figured a pharoah fit the notion pretty well.

Judgmental Attitude = Sanctimony was given as the motivation for Graendal's fall into the Shadow, believing that no one measured up to her standards. Her signature peccadillo is determining what position or place her victims are worthy of.

Lust for Status / Envy = Sammael always thought he deserved more credit and greater honors, and resented the perception of short people as less impressive. I will admit that had there been a Demandred or Bel'al icon, I'd have used that one.

Lust for Power = Lanfear, according to LTT, "loved power" even setting aside her all but visible arousal at the notion of the Choedan Kal.

Sycophantic behavior = I will admit that this image of Narg does not exactly look fawning or servile, but that pretty much describes the entirety of his behavior in his encounter with Rand.

Betrayal of a personal nature = Ba'alzamon was really Ishamael, the Betrayer of Hope

Dishonesty = The Dark One is the Father of Lies

Protagonist Syndrome = I used Jordan, because the affiliated behavior is only appropriate if she is aware of his existence and role in hers.

Hypocrisy = Egwene is the living embodiment of this characteristic

Foolhardiness / Reckless endangerment of herself or others = This was kind of by default, since for once I used the appearance of the image. Foolhardiness is sometimes placed under the category of hot-blooded, and a guy on fire isn't safe to be around.

Flat out incompetence or incorrect conclusions or assessments = In my head, the little blue guy's facial expression indicates he is wondering "why would you think/do that?"

Stupid or Clueless behavior = Self explanatory, I think

Sociopathic mentality or desire toward violence or to victimize others (as opposed to actual action) = I figure the soulless Myrdraal, who get off on torture and rape would be the best expression of such attitudes.

Petty, nasty or spiteful words and attitude / General rudeness = Troll. Again, self-explanatory, IMO.

If your issue is recalling the meaning of each icon applied to each point, I get that. I tried to offer a brief explanation of the less obvious faults, but really the icons are just there to provide some sort of visual touchstone of how many ways a particular thing is bad. You don't need to recall the meanings, to see a whole mess of icons and realize "bitch done fucked up" a whole bunch of times.
