After being gobbled up by a hungry Mashadar, can he be reincarnated? I mean, is there anything left? I know the Jordan quote about Sammael is toast, but does that just mean he's dead, or is he gone and never coming back?

The bit about Mashadar stealing souls comes from Moiraine, who really has no way of knowing this, and who's been wrong about less esoteric things before. However, if you watch Mashadar attack the Trollocs and Myrddraal in The Eye of the World, 'soul stealing' is really not what it looks like. The fog catches the Shadowspawn, and then chokes them. Unless the soul is located withing the stomach or lungs, I doubt there was anything more to it than that.
Besides, wouldn't that mean that thinks like Grey Men would be immune to Mashadar, since they already don't have a soul?
So yeah, I think Sammael could be reincarnated. Whether he has been is a different question though, and while I'm not averse to it, there isn't really much to support it.
The first rule of being a ninja is "do no harm". Unless you intend to do harm, then do lots of harm.
