RJ said that Fain wasn't weakened by the destruction of SL. That implies that Fain wasn't linked to Mashadar (Machin Shin is in the Ways), and we have anyway no reason to believe that. Otherwise Fain should also be able to pose as all the other souls, which Mashadar "ate". And why didn't Fain pose earlier than KoD as Sammael, since Sammael was already killed in the end of ACoS.

IMO, Graendal posed as her old buddy Sammael.
