Here's a list.
Balthamel - decided to torture the Green Man instead of stopping him attacking.
Aginor - overdrew like aNovice. Not even Novices are that stupid.
Be'lal - stopped to gloat, assuming Moiraine couldn't attack.
Asmodean - put up a decent fight. Ironically, performs the best.
Lanfear - assumed everyone dead, ignored them.
Moghedien - stopped to gloat, assuming Nynaeve couldn't attack.
Rahvin - surprised, but had an excuse since he was in TAR.
Sammael - betrayed by Ishamael/Moridin
Osan'gar - forgot that sa'angreal also give you good eyesight. Wasn't as stealthy as he thought.
Semirhage - severely underestimated the force Rand would bring. Betrayed by the Dark One (possibly).
Graendal - didn't expect Rand to balefire an entire fortress to kill her. I don't really fault her, but she does fail as a psychiatrist.
Ishamael - Don't even get me started. At least has the excuse that he wasn't trying to kill Rand.
