I'm just getting back into The Bonehunters (if the next book is this hard to read, I'm quitting the series), and I need to be reminded of a few things:

1. Is the place where Minala/Trull/etc are defending the First Throne that same island where Cutter and Apsalar got separated?

No. That was the Throne of Shadow on Drift Avalii.
This is the First Throne somewhere in the middle of Seven Cities subcontinent.
Tiste Edur want and attack both.

2. How did Trull and Onrack get to said place? The last thing I remember was them interacting with the Tiste Liosian. (spelling?)

3. Who the hell are Monok Ochem and Ibra Gholan? Where did these T'lan Imass come from?

In House of Chains Trull and Onrack were traveling through warrens and time. On they travel they met Monok Ochem and Ibra Gholan (around the Tiste Liosan meeting). Monok Ochem and Ibra Gholan were following the Unbound (false Teblor gods) who were really broken (and therefore rejected) T'lan Imass in search of the First Throne for the Chained God. In the end they all landed in the First Throne room with Minal and the kids and Onrack told them all Midnight Tides.

4. Who in the BLUE HELL is Edgewalker?

Guardian of the realm of Shadow. A very old Ascendant (maybe even forgotten elder God)
