Do they give out honorary J.D.s? :P I thought Fords only significant collegiate accomplishment was winning a couple National "Championships" on Michigans offensive line. I know college football players actually had to go to class back then, but...GERALD Ford? The one who could not even master debarking from an airplane?

Otherwise, yeah, you covered it with references to presidents who do not live in the White House. It should be noted, however, that MLK et al. is often referred to as "the Reverend Doctor." There is no reason multiple titles cannot be mentioned in an honorific, it is just awkward as Hell and non-obligatory, and so uncommon.
