my work-provided coverage sends me a large packet every year that spells out coverages and copays. The same info can be found on their website once you have registered, usually by name and group number and/or policy number. There is also a toll-free number where you can get coverage explanation and limitations, again by providing your group and/or policy number. This was true of Empire Blue Cross and is also true of UHC, the provider we switched to this year. It is as much in their interest as it is in yours to define the coverages and limitations of your policy. Eliminates the time spent on resolving disputes.

I found where to register and can now view a .pdf of the whole policy. Thank you so much. :) Only thing is that it's dated 2007 (the benefits book). Is that still reliable?
