Your best bet would be to pursue various RPG books, outside of D&D for something that is close to what you want, then scan, and edit based off that.

As far as back and front, go with the 2 page format if its necessary. Kind of annoying flipping a page over again. And as people's stats grow it'll lengthen the life of the paper. Both sides aren't being erased continuously.
