I mean, it's really only an issue if you're repeating it for royal blood issues or because a smallish group of people have isolated themselves from the rest of the world geographically or for something like religious reasons.

Even brother-sister incest isn't that big of a deal until a couple of generations goes by. Icky, but not 'deformed babies' icky.
But if two people closely related are both carriers for a negative genetic mutation then that can quickly become a problem for their offspring. So madness could be a recessive trait that became an issue when the Targaryens kept marrying their brothers and sisters to each other.
