At the end of the episode, when the Doctor leaves in the newly remodeled TARDIS, they go back to little Amy sitting outside waiting for the Doctor to return. Then there's the crazy TARDIS noise (you know the one), and THEN they switch to adult Amy sleeping in her bed who hears the noise and wakes up. Which I thought was weird, but it must have been nothing since Amy would have remembered and brought it up instead of harping on the 12 years thing over and over again. But maybe not. I think there's definitely something more going on here - maybe the Doctor came back and told little Amy something, which is the meaning of the whole "remember what I told you when you were seven" thing. I don't know! Exciting!
That the future Doctor reappeared to Amy after the current one left, and told her something, and then jetted off again. As far as little Amy knew, it was the same Doctor, so it still would have been 12 years since she'd seen him.
It's just as likely that the flashback was a dream of him returning, or a memory that got the sound of the TARDIS arriving in the present mixed into it while Amy slept. But it also could be the first thing somehow, because it WAS an odd little bit for them to throw in there, and possibly fits well with remembering what he told her when she was seven.
