You say she's getting raped in order to distract the orderlies so the other girls can steal the objects. How does that work with the mayor? Did the mayor come in and rape her? Were the other girls there at the time? Did they steal the lighter from his pocket while he was at it? Same questions with the cook. If the only answer is "She's crazy so it doesn't make sense," then I'm afraid my opinion about the plot's coherency remains unchanged.

You may not have noticed it, but the actor who played the mayor also played an orderly.
It is not that we don't get all the information since she is crazy, it is that even if we were given the information, we don't know if we can trust it. You are trying to create a sense of the world with firm details when our viewpoint of the world is via its very nature dreamlike and fuddled.
An unreliable narrator means via its very definition that you can't trust what the narrator tells you.
