What I really think is that we meet a younger River who is evil. I bet she's not missing an eye, the patch is for some other purpose, like a mind control device or night vision goggles or something. I don't think River and Mme Kevorkian are the same person. River makes it pretty plain in Ep 7 that she can't interfere in her own timeline.
