I didn't read the Hunger Games, so I can't really comment on this, but I do know that I was very surprised when I suddenly discovered that Tuon, in Wheel of Time, is black, lol. So it is certainly possibly to miss details like that, particularly for readers that are like me and never visualize the characters as more than blurry images.
