My guess is that it was the detonation of Jughead that did it. Ben Linus did talk to his dad about their time on the Island and as members of the DHARMA Initiative. My take would be that the detonation of the bomb set in motion a chain of events leading to the island sinking, but the DHARMA personnel had enough warning to evacuate. That's probably where the 'bleed-through' from one timeline to the next is taking place, with Sawyer being motivated to help Kate despite him being a policeman, because the two timelines are coexisting.

The question mark is Jack's appendix. In the main universe Jack had it removed by Juliet on the Island, but in the Sideverse (congratulations, that's the best name I've heard for it so far) Jack apparently had it out as a child. That could be after 1977 (in the show I think Jack is born circa 1970 or so), but I'm a bit confused on why an Island sinking or not sinking on the other side of the world would have an impact on the timing of Jack's appendicitis. Jacob didn't touch Jack until he was an adult, so that can't be it.

This week is a big reveal episode on several issues, so hopefully that might shed some light on this issue, which is confusing.
