Cyndane - Rand is about to breathe his last breath but Cyndane can't bear to let Lews Therin go. She draws on the source to try to heal him and Alivia wipes her out, thinking Cyndane is about to do something nasty to him - thereby helping Rand die in the process.
i love this one....how about this to add to it - rand and moridin just had their battle and each killed the other. moridin, knowing way more about channeling than rand, did something truly horrific to him/his body, but rand was only able to take the life from moridin. cyndane tries to fix this shizz (for the reasons you said) by putting rand into moridin's body and alivia finshes her off before she can finish the job. maybe nynaeve is there to help out? hehe, ok, i have def entered the realm of straight up speculation....
Demandred - Mat, in a parallel of the way he killed Couladin, who was also focused on destroying Rand.

Slayer - Perrin seems the only likely choice here, which probably means it's someone else entirely. My guesses would be Faile or Hopper.
and i think lan has just as good of a chance as being the man here...
Who will kill each remaining Forsaken ?
