We assume that they engage in rape, but I think it might come closer to a perversion of what we would recognize as sex. Perhaps they mutilate their victim in some way that RJ never wanted to go into more detail. He tends to steer clear of showing how evil might actually be represented, so I think he made a conscious decision to leave this ambiguous.
