While Graendal obviously had multiple opportunities to think of it, she didn't because the author wished to keep up the suspense.
RJ even said he didn't really want to reveal it. BS and Harriet later made the decision to definitely reveal it. I doubt very much that the trademark murder mystery of WoT will be solved in an ebook prologue.

That some people always argue that Slayer's single short PoV in WH makes it impossible that he was the killer, while Graendal can of course have lots of PoV without any revelation but still be the most likely killer in their eyes..
