The Shadow punishes failure and betrayal very severely, and I think she wanted to be dead and well along her way to Rebirth in the soul pool way before any Black Ajah or Forsaken could tell the Dark One what she did. If she was lucky, the souls of both she and Tomas were being spun back into the Wheel before the DO could grab them.
