For me it's Therava. She's just so utterly pitiless. And unlike pretty much every other BA or Forsaken or Seanchan antagonist, there's no arrogance to her; she really is as hard and cruel and clever as she thinks she is.
I don't think we've ever seen any indication of her being clever. Forceful, scary, ascetic, intent, bondage lady..But she hasn't done anything actually intelligent. She did try to get one over on Sevanna, but still.
But you're right that Therava is scary, I think so too. I think the new Padan Fain (in ToM) may well be the new scariest character. Prior to reading the prologue I thought he was just a dull shadow wannabe.
A shame that these minor characters are the most frightening, that they have this human evil that is scary. The Shadow/Dark One/Ishamael should have been terrifying, but isn't. The DO is an impersonal entity and that isn't frightening. Ishamael could have been deeply creepy (a mad nietzscheistic (??) philosopher) but he isn't, at all.
