Mesaana is not dead, she is alive in the Tower with the mind of a child. Do you think that by keeping her alive in this state, she is denying the DO access to her, and denying his ability to reincarnate her? (albeit unknowingly, on Egwene's part)

I'm pretty sure the DO can send Shadar Haran, Slayer, or some other lackey to end Mesaana. He just chooses not to at this juncture, as soul transmigration takes time and resources.
A broken mind doesn't = broken soul. If Mesaana was to be reincarnated into a different body, I'm confident her mind will be restored, provided the body she transmigrated into isn't brain-damaged.
