Sanderson has just posted about this and said the "hard core fan freaks" would know who the extra person is and why they were in that artwork. I figured this would be the best place to ask.
If I recall correctly, at one point another youngster was going to leave the Two Rivers, but not be ta'veren. Obviously not essential for any plot reason and cut, but looking back, IMO would have been a big plus to have us get to know another TR youngster over TEOTW and then have them die at the Eye. Maybe would have removed that 'only red shirts die' feel that still persists.
