Gratuitous deaths do not make a good story, any more than giving main characters plot armour.

Character death should occur when it is valuable to the story.

Either one is amateurish, IMHO. It's less about "realism" than insecurity and gimmicks. The story should stand on its own without the need for sensationalist hooks; rape scenes may guarantee ratings for your TV show or make your book a best seller, but just because controversy sells doesn't make it worth my time or money.
