Jordan said it's Mashadar that killed Sammael and got his soul. It sounds like Rand's balefire missed him or was too late.
Another ironic death.. Sammael, the methodical defender, got stabbed in the back because he failed to prepare proper defensive positions against Mashadar in SL, and saw his own deathtrap snap on himself.
