Remember though, that Moridin showed Graendal this prophecy at the end of the Prologue, insinuating that if Perrin was going to die in fulfillment of said prophecy, it would happen in that volume of the series. She certainly tried hard enough to make it happen and at the end, she was being 'punished' by Shaidar Haran for her failure, implying that the opportunity had passed.

So I think this prophecy is already, at least, partly broken. Does that then invalidate the rest of the prophecy? I don't know...
