If I am not mistaken, it is referred to in each of the various POVs and is one of the markers used to reconcile all the timelines. As for what caused it I don’t think a definitive answer has ever been said. Operation from memory, I think RJ RAFOed it in a Q&A. Maybe it was the Natrim Barrows balefire but propagated backwards, but I have always thought of it as simply the DO touching the pattern.
