Graendal was punished for not getting Perrin killed in the Towers of Midnight. Not sure if she was killed like Lanfear then transmigrated or that her soul was just ripped from her body and stuffed in that ugly hag.

That would probably explain it. Unlike the books RJ authored, I only read Sanderson's offerings 1 time through, so I'm a little vague on some of the more recent events.
