There's an editor, but more importantly isn't there a dedicated 'continuity' assistant? This is EXACTLY the thing said person should be picking up on.
To be fair to BS, he shouldn't have to stop after every paragraph and check he's got the nuances down 100%. That'd quickly wreck any groove of writing the story. These should be picked up by 'Team Jordan' so these little 'glitches' are corrected before it becomes a final manuscript. Heck you could put a final draft out to 10 people here or on Dragonmount and 90% of these type ones would be picked up.
Hopefully for ToM and AMoL as well as ensuring 'big picture' continuity, there's also more paid attention to the 'little picture' ones.
