Deeper in the Blight than Malkier, beyond and below the mountains at its northern borders, not far from SG.

This "fortress" is "Ishamael's palace" (when Rand visits Moridin, he recognizes the place from EOTW. It sounds like Graendal had never visited it before TGS (except from TAR, possibly), like Moghedien when she was brought there to meet Moridin had not). Moridin just seems to be fortifying it now. It seems located somewhere near/below the eastern opening of the Valley of Tankhan'dar, above the northern opening of Tarwyn Gap. From the Light's POV, it blocks the way to Tankhan'dar.
