We went to the library, chose a book, read it, did a report on it (with a lot of vocab cards and other things we had to do). I think we had to do 4 per year minimum, or maybe 8.
The only problem with the format was that we were so open to choose books we'd get ourselves in trouble. I once chose War and Peace ... as a seventh grader. I couldn't handle it and ended up getting in trouble because I never did a book report or anything on it.
