Since he is supposedly the original Dalai Lama reborn I would assume he will continue to be the Dalai Lama as long as he lives. Will they have to choose a new Dalai Lama in Tibet and then sneak him out of the country?
No.
The current Dali Lama has said that if he dies in exile then the next Dali Lama will not be born in a land under Chinese control. Of course China is already prepared to name a Dali Lama it can control so there will be two I imagine after the current one dies.

It's funny how we hate all other unelected, theocratic leaders, but not this one.
