1. Have you ever fallen in an embarrassing way? (tell the story)
When I was in 2nd grade we were on a field trip in the middle of the woods the day after it rained really hard, and we were walking up a hill. My teacher announced to the class "Be careful here, it's slippery" and about two seconds later she fell and pulled me down with her. That was much more embarrassing for her, I think.
I once fell while running up the steps at Borders. That was probably the most embarrassing. I just got up and walked quickly up the stairs and didn't look back to see how many people were watching.
Just remembered another. Last year in the parking lot at school. There was a really bad patch of ice and I fell completely, and this really old custodian guy asked me if I was okay. As I opened the door to go inside I heard him talking about my fall to one of his co-workers.

That would totally embarrass me! Glad to know you weren't really hurt in any of those.
The last one sounds like an interesting scene in a story. you should write that. ^_^
