2) incorrect use of the nominal case for personal pronouns when the direct object or object of a preposition (e.g., "Between you and I..." )
This is big on my list too. Apart from the preposition issue, I find it even worse when people try to make a combination like that possessive...I actually hear people saying things like "it was her and I's idea."
