Employers don't have to pay, and the self-employed, employed, and unemployed are all put on a level playing field. There's a tax deduction for health insurance that could be tweaked to provide a 100% deduction for the cost of primary health insurance up to a limit. That way the only disadvantage would be that people employed by others pay a bit more in FICA. Boo hoo.
