All it requires is enough instability in a nuclear power for long enough for a bomb to be sold to terrorists and dropped into a container on a cargo ship. Or for one ICBM in one country to be hijacked. Currently there are only a few countries that people have to worry about, but if the North Korea situation isn't resolved, or if Iran is violating their agreement as we speak, the risk spikes.
