When I first visited Labyrinthian, I drew the attack of a Frost Troll from the font. As I was fighting it, I wondered where in the hell was Lydia, and then I heard fighting from behind and the roar of another Frost Troll. I finished mine and turned to help Lydia. Her Troll had already closed on me and she was laying on the ground... dead and not getting up. I finished the second troll, and Lydia was indeed dead... and I know for a fact that I didn't hit her with anything.
I reloaded the save and we soldiered on... damn Frost Trolls are not about to kill MY house carl.
