I was shocked about the ending (spoilers), with the ending of the telepathic precognition duel. I was shocked for the ending was extremely close to the ending of the "The Final Problem," yet it was different enough that you don't realize what was about to happen till Holmes blows his tobacco into Moriarty face.
Adding a castle with a balcony instead of a ledge makes you forget that there is a waterfall underneath. (There was no castle in the original short story).
