The Land of Madmen has never been mentioned in the series, unless it was in tGS and I've forgotten.
The only time it's been mentioned is the BWB. I'm guessing 95% of WoT readers aren't aware of the Land of Madmen.
People complain in books when someone/something suddenly comes along out of nowhere to save the day without any set up. I think it should go the same way with bad guys ruining the day, and Demandred suddenly showing up with a ton of channelers from the Land of Madmen would be the same thing.
That's why, between Land of Madmen and Shara, Shara is much more likely. We've heard about Shara several times. From the Jainfarstrider book early on, to the silk from east of the Waste, to Graendal's pets.

If Demandred isn't Roedran (or controlling Roedran)

Lol. So Slayer is the easy solution to every single mysterious death in the series? Dull as hell *NM*

Fain playing a significant role in the LB would too much allude to LotR, IMO. *NM*

I agree. I think this refers to The Great Battle done but the world not yet done with battle

They followed Fain all the way to Caemlyn and shot arrows at Rand

Murandy just doesn't fit Demandred. Also, Sammael could have been wrong.

Sammael probably refered to the Dragonswron in the south of the Westlands...
