Min actually states that the only viewing she ever got wrong was that Rand could not succeed without a woman dead and gone. IIRC she never once thinks about Thom and Moiraine.
"Rand will die without Moiraine."
This viewing CANNOT fail. It isn't possible. If Moiraine dies, then the viewing comes true. If she lives, the viewing comes true. It's just a statement. He'll die if Moiraine dies. It's 100% impossible for this viewing to fail. It's like saying "The grass will grow if it rains." If it rains it will grow, if it doesn't rain it won't grow. It can't fail. It's correct no matter what happens. Therefore, this is not the viewing that Min is thinking of.
What failed is a viewing Min had about Moiraine doing something that she hasn't done yet, which is impossible to do if she's gone. There's nothing about the dead and gone viewing which is impossible. As far as Min knows the viewing is still accurate. Moiraine is dead, Rand will fail, the viewing is correct.
