Granted this was "the Age of Legends" and all, but keep in mind that's compared to the present state of randland. We don't really know just how advanced they were, or how "perfect" their technology was, and how much of what we're told is distorted by time and/or bias.

I really can't think of a single thing that can be done to the human body that doesn't have some sort of side effect, however minute. I do not see why this would be any different.

The Oaths place a magical bind on a person's mind. The magic to do so would have to be perfect to cause no side effects. It's amazing that it's limited to the ageless look and not living as long.
